Long Term Conditions
Assistance for clients with long-term health conditions such as diabetes, heart disease, respiratory illness, or cancer.

Neurological conditions
Support for individuals with neurological conditions (ie., stroke, multiple sclerosis, dementia, Parkinson’s disease, spinal injuries).

Post Hospital Discharge
Post-hospital or rehabilitation support for individuals recovering from surgery, injury, or acute illness.
Our Complex Care provision includes, but is not limited to:
Medication administration and monitoring in line with care plans and clinical guidance.
PEG feeding, catheter care, stoma management, and other specialised healthcare tasks (where trained staff are authorised)
Support with mobility, including the use of hoists, wheelchairs, and other assistive devices.
Collaboration with multidisciplinary teams (nurses, occupational therapists, and GPs) to ensure continuity and safety of care.
Ongoing risk assessments and reviews to ensure care plans remain up to date and effective.
Dementia & Alzheimer's Disease
Dementia, including Alzheimer’s disease, is a progressive condition that affects memory, thinking, behaviour, and the ability to manage everyday tasks. As the condition develops, individuals may experience confusion, communication difficulties, changes in mood, and increasing reliance on others for support. These changes can be distressing not only for the individual, but also for their families. We understand how challenging living with dementia can be for both individuals and their families. That’s why we’ve carefully developed our specialist Dementia and Alzheimer's Care Plan. Our carers provide calm, patient, and consistent care, focusing on familiar routines and meaningful engagement to help reduce anxiety and maintain a sense of stability. We support individuals to remain in their own homes for as long as possible, promoting dignity, reassurance, and quality of life while working closely with families to adapt care as needs change.

Stroke Care
A stroke occurs when the blood supply to part of the brain is disrupted, which can result in a range of physical, cognitive, and emotional challenges. Depending on the severity, a stroke can have a significant and lasting impact on a person’s life, often affecting mobility, speech, memory, and their ability to carry out everyday activities independently. These changes can be both physically and emotionally challenging.Recovery can vary greatly from person to person and may take time which is why we work closely with individuals, families, and healthcare professionals to encourage rehabilitation, Through person-centred care, we aim to support recovery help individuals regain confidence and independence in their own homes.

Multiple Sclerosis (MS)
Multiple Sclerosis (MS) is a long-term, progressive neurological condition that affects the central nervous system, disrupting communication between the brain and the body. It can cause a wide range of symptoms, including reduced mobility, poor coordination, muscle weakness, fatigue, pain, and cognitive changes. Symptoms often fluctuate and may worsen over time, meaning an individual’s care needs can change from day to day or progress gradually. We work closely with individuals and their families to promote independence and provide consistent emotional reassurance. It’s estimated that over 150,000 people in the UK are living with MS.
Around 7,100 new diagnoses occur each year.

Our carers understand the importance of patience, timing, and consistency when supporting someone with Parkinson’s. Care is tailored around individual routines and medication schedules, with support provided for mobility, personal care, and daily activities. Through a calm, respectful, and person-centred approach, we help individuals maintain independence, dignity, and confidence within their own homes.

Spinal Injuries
Spinal injuries can be life-changing and may affect a person’s mobility, sensation, and ability to carry out everyday activities independently. The impact of a spinal injury varies depending on the location and severity of the damage, and may result in partial or complete paralysis, chronic pain, muscle weakness, or difficulties with bladder and bowel control. These changes can place significant physical and emotional demands on both individuals and their families. Our carers take the time to understand how each spinal injury affects the individual day to day. Support is delivered with a strong focus on safety and comfort, whether that involves assisting with movement, maintaining correct positioning, supporting skin integrity, or helping someone use specialist equipment confidently. What is a care assessment?
A care assessment is a discussion and review of your individual needs, preferences, and health requirements. It helps us create a care plan tailored specifically to you.
Where can my care assessment take place?
Your assessment can be conducted wherever is most convenient: usually in your home, a family member’s home. Home assessments are preferred as they give the best view of your daily environment.
Who conducts the assessment?
A Registered Care Manager or senior member of our team will carry out the assessment. They have experience in evaluating care needs, safety, and support requirements.
How long does a care assessment take?
Most assessments take between 45 minutes and 1.5 hours, depending on your needs and any additional questions or planning required.
Is the care assessment free?
No. Your care assessment will be £75 with no hidden fees. This allows you to understand your options without any financial commitment. However you intital Consultation is free.
What information will I need to provide?
You may be asked about your daily routines, health conditions, medication, mobility, personal care needs, and any preferences for companionship or lifestyle support.
What happens after the assessment?
After the assessment, we create a personalised care plan and discuss the best options with you and your family. Once agreed, care can start as soon as you’re ready.

How It Works
Our service can provide support for a few days, weeks, or even months. Whether your loved one needs extra help after hospital discharge or you require a break as their primary caregiver, we’re here to deliver the care they need, exactly when it’s needed.
Our experienced carers can assist with personal care, meal preparation, medication management, appointments, outings, and companionship. You set the care plan, and we ensure it is followed meticulously to meet your loved one’s specific needs.
